Access Request Form Instructions:
Before you begin:
You will need the following information to complete your Access Request form.

Onsite Access (ID Badge) with or without computer access

- Your own valid email address. You cannot use a shared email address or you will be unable to complete this process
- The location you will be working. Please check with you Fermilab point of contact if you are unsure
- The name, email address, and phone number of your Fermilab point of contact
- Your Purchase Order (PO) number (Contractors only) Please check with your point of contact for this information
Notes About Onsite Access
- Up to one year of access can be requested
- If you are planning to come onsite at all during your requested access, select the onsite option on the Access Request form
- You are required to present a valid REAL ID in order to come onsite
*Notes for onsite foreign nationals…
If you are on a short-term visa (i.e. B-1, VWB, etc.) and will need a long-term badge, you can put the full date range that you need an ID badge for and then we can issue multiple badges (if needed) within that timeframe without needing you to re-submit a ticket/form.
Remote Access (ONLY computer accounts)
- Your own valid email address. You cannot use a shared email address or you will be unable to complete this process
- The name, email address, and phone number of your Fermilab point of contact
- Your Purchase Order (PO) number (Contractors only) Please check with your point of contact for this information

How to Report Concerns or Violations
REAL ID at Fermilab
Please note that beginning July 1, 2021 personnel requesting access to non-public areas of the Batavia and South Dakota sites must submit a REAL ID-compliant document as part of the badging process. Additional information regarding REAL ID can be found here.
Onboarding
Onboarding Instructions
Once your ticket is approved, you will receive two emails from Fermiworks/Workday with your username and password to complete your onboarding. You must complete onboarding with in 30 days or your access request will be canceled.
If you do not receive one of the emails or have trouble logging in, please contact the service desk at 630-840-2345. If you have previously logged in to FermiWorks, you can also try clicking the “Forgot Password” link and see if you can reset your password that way.
Below are general instructions (with screenshots) on how to Complete Onboarding in FermiWorks, there my be some variation.

Training
Training Instructions
Before your onsite access to the lab is granted, you will need to complete any relevant training.
Contractors
Contractors working onsite are required to take a safety training and receive their “blue card”, which is valid for 2 years. If you do not have a valid blue card, please contact gert@fnal.gov to sign up for a training. Training is held Monday through Friday at 7:30 and can be done on the day of your badging appointment. You can also visit ESH subcontractor orientation class/ for additional safety training information.
You must bring your blue card to your badging appointment.
Affiliates and Users
Affiliates and Users have three required trainings prior to being badged or granted computer access. Please review the following trainings.

Instructions for completing the Sexual Harassment Awareness test:
DO NOT CLICK ON THE TEST LINK AT THE END OF THE SEXUAL HARASSMENT PRESENTATION
- Go to the ESH&Q Testing Center
- In the drop-down menu under “Select an online test,” select one of the following classes:
- “Sexual Harassment Awareness and Prevention for Fermilab Users, Visitors and Contract Employees [FN000531/CB] 1885” (double check to make sure you have the right class number on this one because there is more than one “Sexual Harassment” training listed)
- In the box under “Enter your Fermi ID,” type in your Fermi ID number.
- Click on the button to the left of “Visitor” (under “Enter your Fermi ID”)
- Click on the “Submit” button
- On the following page, your email address will be displayed in bold type; verify that the email address is correct.
- If the email address is incorrect, contact us ASAP to correct it
- If the email address is correct, then click on the “Request Test” button
- A link to the test will be emailed to you shortly.
- Log in to your email and click on the link that is sent to you.
- Follow the instructions on the test link to complete the test.
