Access Request

Before you begin the access request form

To all those completing the form, please have the following ready to complete your access request form:

  1. Scan of your photo ID (i.e. Passport or REAL ID driver’s license)
  2. If you are a non-US citizen, Curriculum Vitae (CV)
For those requesting onsite access (ID badge) with or without computer access

Please have the following ready:

  • Your own valid email address. You cannot use a shared email address or you will be unable to complete this process.
  • The location you will be working. Please check with you Fermilab point of contact if you are unsure.
  • The name, email address, and phone number of your Fermilab point of contact.
  • Subcontractors only: Your purchase order (PO) number. Please check with your point of contact for this information.
Notes about onsite access
  • Up to one year of access can be requested.
  • If you are planning to come onsite at all during your requested access, select the onsite option on the access request form.
  • You are required to present a valid REAL ID in order to come onsite.
Notes for onsite foreign nationals

If you are on a short-term visa (i.e. B-1, VWB, etc.) and will need a long-term badge, you can put the full date range that you need an ID badge for. The Campus Access office can issue multiple badges, if needed, within that timeframe without needing you to re-submit an additional request form.

For those requesting remote access (computer accounts only)

Please have the following ready:

  • Your own valid email address. You cannot use a shared email address or you will be unable to complete this process.
  • The name, email address, and phone number of your Fermilab point of contact.
  • Subcontractors only: Your purchase order (PO) number. Please check with your point of contact for this information.

REAL ID at Fermilab

Since July 1, 2021, individuals requesting access to non-public areas of the Batavia and South Dakota sites are required to submit a REAL ID-compliant document as part of the badging process.  Additional information regarding REAL ID can be found on our web page.

 Translate the page

Training

Training instructions

Before your onsite access to the lab is granted, you will need to complete any relevant training.

Contractors

Contractors working onsite are required to:

  1. Take a safety training and
  2. Obtain their “blue card”*
    • The “blue card” is valid for 2 years.
    • If you do not have a valid blue card, please contact gert@fnal.gov to sign up for a training at the Batavia, Illinois site.
    • Training is held Monday through Friday at 7:30 in the Fermilab Welcome and Access Center (Aspen East) and can be done on the day of your badging appointment.
    • Contractors located in South Dakota will complete training onsite in South Dakota
    • You can also visit ESH subcontractor orientation class for additional safety training information.

*It is a requirement to bring your blue card to your badging appointment.

Affiliates/Users

Affiliates/users are required to complete three different trainings prior to being badged.

  1. New User/Affiliate Orientation
    • Click on the test link at the end of this presentation
  2. Workplace Violence and Active Shooter/Active Threat Awareness Training
    • Click on the test link at the end of this presentation.
  3. Sexual Harassment Awareness and Prevention Training 
    • To complete the Sexual Harassment training you must log-in with your Fermi Services account.

Instrucciones de entrenamiento

Antes de que se le otorgue acceso al laboratorio, deberá completar un entrenamiento.

Contratistas: 

Los contratistas que trabajen en el sitio deben:

  1. Completar un entrenamiento de seguridad para poder obtener su “tarjeta azul”
    • La “tarjeta azul” tiene una validez de 2 años.
    • Si no tiene una tarjeta azul válida, comuníquese con gert@fnal.gov para inscribirse en una capacitación en el sitio de Batavia, Illinois.
    • La capacitación se lleva a cabo de lunes a viernes a las 7:30 de la manana en el Centro de Acceso y Bienvenida de Fermilab (Aspen East) y se puede realizar el día de su cita para la acreditación.
    • Los contratistas ubicados en Dakota del Sur completarán la capacitación en el sitio en Dakota del Sur.
    • También puede visitar la clase de orientación para subcontratistas de ESH para obtener información adicional sobre capacitación en seguridad.
    • Es requisito traer su tarjeta azul a su cita de acreditación.

Afiliados/Usuarios

Los afiliados/usuarios deben completar tres entrenamientos diferentes antes de recibir la insignia.

Appointment

Making a badging appointment

You are ready to make an appointment for a Fermi-ID Badge if you have completed the following:

  1. Submitting an access request form
  2. Onboarding in FermiWorks (if your request was submitted before December 14, 2023).
  3. Required trainings (completed or scheduled)
