Documents required for on-site access

You must be on-site in order to obtain a Fermilab ID badge.  Once you arrive on-site (or if you are currently here and need to extend an existing ID badge), bring the following documents to the Users Office.  The Users Office is located on the mezzanine level of Wilson Hall and is open Monday-Friday, 8:00-4:30.


PLEASE DO NOT EMAIL YOUR DOCUMENTS TO US.  WE MUST SEE ORIGINAL DOCUMENTS (NO COPIES)

It is against our computing policy to send/receive emails that have PII in them (personally identifiable information).  We also must see original documents; copies are not acceptable.  The only exception to these two rules is medical insurance, as long as it does not contain PII such as a date of birth, citizenship, etc.

If you are working on an experiment that is associated with Fermilab’s Accelerator Complex or CMS, your home institution must have a signed Non-Proprietary User Agreement on file with us.

US citizens

  • Valid government issued photo ID (i.e. driver’s license or passport)
  • Proof of medical insurance coverage (i.e. medical insurance card or certificate)

US legal permanent residents

  • Valid government-issued photo ID (i.e., driver’s license or passport)
  • LPR / green card
  • Proof of medical insurance coverage (i.e., medical insurance card or certificate)

Non-U.S. citizens

  • Passport with visa stamp in it
  • Visa (if other than an ESTA/visa waiver)
  • I-94 (see instructions below on how to print this)
  • Other pertinent visa documents including, but not limited to, a DS-2019 form, I-20 form, I-797 form, EAD card.
  • Proof of medical insurance coverage (i.e., medical insurance card or certificate)

How to find and print your I-94

  • Go to https://i94.cbp.dhs.gov/I94/consent.html
  • Read the Security Consent page. Then at the bottom, click on the circle next to “Yes, I have read and understand the information and agree to these terms.”
  • Click on the “Submit” button on the bottom of the page.
  • On the next page, enter the information in the following fields exactly as it appears on your passport:
    1. Last/surname
    2. First (given) name
    3. Birth date
    4. Passport number
    5. Country of issuance
  • Click on the “Get Most Recent I-94” button to generate your I-94.
  • Print the I-94 page.

High School Students / Under 18 Years of Age

  • In addition to the documents listed above, users under 18 years of age will also need a signed Student Registration form
  • Workers under the age of 18 are prohibited from certain activities.  Please reference FESHM 1010 for more information.
