Documents required for on-site access

Certain documents are required to obtain on-site access. Please refer to the information on this page prior to your badging appointment. Below are important notes about documentation.

  • You are required to make a badging appointment at least one day in advance to obtain your Fermilab ID badge.
  • Please bring original documents to your badging appointment. The Campus Access Office is required to review original documents to issue your Fermilab ID badge. Copies are not acceptable.
  • Please do not email documents with personal identifiable information (PII) such as date of birth, citizenship, etc. Sending and receiving PII documentation is prohibited per the Fermilab computing policy. Exceptions include:
    • Medical insurance: If your medical insurance does not contain PII it can be emailed.
    • I-20 form or a DS-2019 form: If these document types were only provided to you via email, they can be emailed.
  • To obtain a Fermilab ID badge, all workers must present a REAL ID-compliant identification document.
    • Examples include a REAL ID driver’s license, passport, LPR/green card, HSPD-12 badge (TWIC cards) or an EAD card. Please visit the REAL ID at Fermilab page for more information.
    • A REAL ID-compliant identification document must be brought to every badging appointment regardless of whether it was previously scanned by the Campus Access Office.

Required documents

  1. Valid REAL ID-compliant identification photo ID
  2. Fermilab ID badge (if issued previously)
  • Users and affiliates only: proof of medical insurance coverage (i.e. medical insurance card or certificate)
  • Subcontractors only: blue card from contractor orientation
  1. Valid government-issued photo ID (i.e., driver’s license or passport)
  2. LPR/green card
  3. Fermilab ID badge (if issued previously)
  • Users and affiliates only: proof of medical insurance coverage (i.e., medical insurance card or certificate)
  • Subcontractors only: blue card from contractor orientation

If you are a non-U.S. citizen, you must check in with the Fermilab Visa Office prior to your scheduled badging appointment if:

  • You are a Fermilab-sponsored J-1 exchange visitor arriving for the first time to begin your J-1 program, or
  • You are a new non-U.S. citizen employee.
  1. Passport
  2. *Visa stamp or VWB stamp.
  3. *I-94 (Please note that this document is only available after you enter the United States)
  4. Other pertinent visa documents such as the DS-2019 form, I-20 form, I-797 approval notice (for H-1B or L-1) or EAD card
  5. Users and affiliates only: proof of medical insurance coverage (i.e., medical insurance card or certificate)
  6. Subcontractors only: blue card from contractor orientation
  7. Fermilab ID badge (if issued previously)

*To obtain your Fermilab ID badge, you must be admitted with business status (i.e., VWB/business, B-1/business) and not tourist status (VWT/tourist; B-2/tourist).

Tourist status is acceptable for authorized guests. Other exceptions include:

  1. DACA status
  2. H-1B, if you changed your status to H-1B while in the United States and, therefore, may not have an H-1B visa stamp in your passport.

I-94 document

The I-94 document is proof of legal visitor status for non-U.S. citizens and is required to issue a Fermilab ID badge or temporary visitor badge. The I-94 will only be available after an individual enters the United States and may take up to 24 hours after arrival. How to find and correct the I-94

  1. Before your badging appointment, access your I-94 on the U.S. Department of Homeland Security website.
  2. Read and acknowledge the “Terms of Service”.
  3. Enter your traveler information exactly as it appears on your passport:
    • Last/surname
    • First (given) name
    • Birthdate
    • Passport number
    • Country of issuance
  4. Click “Next” to generate your I-94.
  5. Review your I-94 to ensure that the business status is correct and date of entry is the most recent.
  6. If there are no issues, print the I-94 page.

If your I-94 is incorrect or missing from the Customs and Border Protection, email the CBP at i94chi@cbp.dhs.gov.

  • If you call, you will most likely be instructed to send an email.
  • CBP contact information:
  • You must contact the CBP to get the document fixed. Fermilab’s Global Services Department is unable to contact the CBP on your behalf.
  • Please either cc Campus Access or forward the email that you send to CBP to Campus Access.
  • Global Services is unable to issue a Fermilab ID badge or allow you onsite until we receive proof that you emails CPB for correction of your I-94 document.

High school students or individuals under 18 years of age

In addition to the required documents listed above, users and affiliates under the age of 18 will also need to complete and submit a signed student registration form. This form can either be uploaded when submitting the access request form, or a hard copy can be brought to your badging appointment with you.

Workers under the age of 18 are prohibited from certain activities. Please reference FESHM 1010 for more information (authentication required).
